Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced case management visit to inquire on an incident report sent to licensing on 04/22/26 by Inland Regional Center. LPA was greeted and explained the reason for the visit to Staff, Michael Collins. Michael phone called Administrator, Maria Collins and informed her about the purpose of the visit.
On 03/09/2026, it was reported that client 1 (C1) accused staff 1 (S1) of sexually assaulting them.
During today's visit, LPA spoke with the administrator over the phone to discuss the incident and surrounding event. LPA did a health and safety check and reviewed records. During record review, it was discovered that an internal investigation facilitated by law enforcement resulted with S1 cleared from the allegation. Per record review, S1 was cleared to go back to work at the facility on 3/12/2026.
No deficiency was issued during this visit. An exit interview was conducted where this report LIC809 was reviewed, discussed and then provided to Staff, Michael Collins.
