Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to a Special Incident Report (SIR) submitted to the Community Care Licensing Office on 01/7/26. LPA was greeted and met by Case Manager, Geraldine Cooper. LPA introduced self and stated purpose of the visit.
Per the SIR, one client fell backward on their electric wheelchair in the van's lift while at a community outing at Target.
During today's visit, LPA met with Geraldine to discuss the incident and surrounding events. LPA conducted a phone interview with Director of Regional Administration, Rosalind Ford and records review. During interview, it was revealed that staff did not follow training procedures when assisting a client using a wheelchair inside the van and caused them to fall backwards on the van's lift.
One deficiency was citedd during today's visit. An exit interview was conducted where a copy of this report LIC809, LIC809D and appeal rights were provided to Geraldine Cooper.
