Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to an incident that was reported to Community Care Licensing on 1/20/26. LPA was greeted and met by Administrator, Lorida Martin at the front door. LPA introduced self and stated purpose of the visit.
During today's visit, LPA discovered that one client moved out on Sunday 2/1/26 and was transported by support living services. LPA inspected the client's bedroom and reviewed facility records. LPA observed that the client took all of their belongings and cashed out their P&I.
No deficiency was issued during this visit. An exit interview was conducted where this report LIC809 was reviewed, discussed and then provided to Staff, Lorida Martin.
