The inspector’s account

Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to an incident that was reported to Community Care Licensing on 11/3/25. LPA was greeted and met by Administrator, Lorida Martin at the front door. LPA introduced self and stated purpose of the visit.

During today's visit, LPA discussed the incident and surrounding events with Staff, Lorida Martin and Administrator, Cielita Ravelo. LPA reviewed facility records. During interview and records review, it was discovered that on 10/10/25, client 1 (C1) left the facility at 9:38am and returned back at 10:45pm accompanied by a law enforcement officer. On 10/12/25, C1 disclosed to staff that the reason they returned late on 10/10/25 was because they were sexually assaulted. Ravelo stated that Inland Regional Center was conducting an investigation and the facility did not have details because C1 wanted to keep it confidential. Ravelo stated that the sexual assault occurred at a public place by an unknown individual. Ravelo stated that C1's CSC implemented a rule for C1 to return back to the facility by 8pm when they are out in the community or to call the facility by 8pm and report their whereabouts. Per C1's physician's report and Individual Program Plan, C1 is allowed to go out in the community unsupervised.

No deficiency was issued during this visit. An exit interview was conducted where this report LIC809 was reviewed, discussed and then provided to Staff, Lorida Martin.
