Licensing Program Analyst (LPA) Michelle Echeverria arrived unannounced to conduct a health and safety case management visit to the facility. LPA met with Staff, Mayanne Matias and introduced self and stated purpose of the visit. LPA was informed that there are currently 4 residents in care who are in the facility.
During today's visit, LPA conducted a health and safety inspection, reviewed records, and did a walk-through of the facility. LPA found no current issues.
An exit interview was conducted where this report, LIC809 was discussed and provided to Administrator, Cristina Duarte who later arrived.
