Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria conducted an announced case management visit. The purpose of the visit was to conduct a pre-licensing inspection, due to a request for a capacity increase of 25 individuals. Upon arrival LPA met with Assistant Program Manager, Carlos Diaz and a few minutes later with Administrator, Lala Jones. On 12/03/25, the San Bernardino County Fire Department approved the facility for 70 ambulatory and 15 non-ambulatory persons, for a total capacity of 85. LPA conducted a facility tour with Administrator, Lala Jones.
The site consists of a large building with services provided to clients in the first floor. The second floor consists of personnel offices not licensed by Community Care Licensing. Day Program has 11 program rooms for computers, recreation, music, arts & craft, activity and exercise rooms, etc. The adult day program has 3 bathrooms, and two staff offices. Storage areas were also inspected, chemicals and cleaning agents were locked during visit. Smoke detectors are hard wired and were tested by Victorville Fire Department on 5/12/25. Furniture; tables and chairs, refrigerators, microwave and other equipment were all in good repair. Water temperature was measured at 118 degrees Fahrenheit. Facility does not handle money and medication for clients. Complete first aid kit, including manual, client and staff records will be kept in locked inside the supervisor's office. A variety of snacks were available including water, during the inspection. The facility was clean, equipped with the necessary equipment and able to accommodate the requested capacity.
The Physical plant capacity increase is noted with the additions, all necessary documents have been received and a new license reflecting the capacity increase will be issued.
An exit interview was conducted and a copy of this report was provided to Assistant Manager, Carlos Diaz.
