The inspector’s account

Licensing Program Analyst (LPA) Michelle Echeverria conducted an announced visit to confirm that there are no residents at the home for the facility's closure. LPA was granted entrance by Licensee/Administrator Joyce Dechimo and partner Christopher Reulbach.

LPA was informed that there are no residents living in the facility. The licensee is initiating this closure. LPA inspected the facility which included the bedrooms, bathrooms, dining area, living room, kitchen and garage. LPA observed that construction was completed and the kitchen and living room were renovated after the fire. There were no residents present, and there were no belongings of residents in the facility. Moreover, Licensee/Administrator stated that the reason for the closure was due to medical issues. Licensee was not able to surrender the license to LPA because it was burned during the fire that occurred on July 12, 2024.

The effective date of closure will be 1/30/2026.

An exit interview was conducted where this report, LIC809 was discussed with and provided to Joyce Dechimo.
