Facility Evaluation Report
On 11/18/2025 at 2:15PM Licensing Program Analyst (LPA) Renese Howell-Small arrived to the facility unannounced to follow-up on incident reports that were submitted to Community Care Licensing (CCLD). LPA was greeted by a staff and gained entrance into the home. LPA met with Licensee, Olayinka King and stated the purpose of the visit. LPA confirmed that there is currently one (1) client in care.
LPA conducted a health and safety check on Client 2 (C2). LPA interviewed two staff and one (1) client. LPA obtained supportive documentation. No deficiencies were cited during this visit.
An exit interview was conducted where this report LIC809 was discussed and copy provided to Licensee, Olayinka King.
