Facility Evaluation Report
Licensing Program Analyst (LPA) Magda Malcore conducted an unannounced required Annual Inspection. LPA identified self to Program Director, Therese Kragness who was advised of the purpose of the visit. The facility is currently licensed as an Adult Day Program with an enrollment of 69 clients. LPA was accompanied by the Program Director to conduct a general overall inspection, which included, but was not limited to, the following:
LPA inspected the facility inside and outside. All passageways are clear of obstructions. The facility is maintained at a comfortable temperature of 72 degrees fahrenheit. Client activity rooms were equipped with required furniture, supplies and space for clients. Client bathroom appliances were operating in good and sanitary conditions. LPA observed grab bars in the bathrooms. LPA observed sufficient furniture and lighting throughout the facility. The hot water temperature tested within regulation at 105 and 106 degrees fahrenheit. There are no firearms/ammunition or bodies of water. Clients are responsible to bring their own lunch. Snacks are made available. Staff present were sufficient for the number of clients in care. LPA observed fire extinguishers, smoke alarms, carbon monoxide alarms, client personal rights posting, facility license and first aid kit in the facility. The facility maintains an infection control plan, staff fire drill training, and emergency/disaster plan for review.
LPA reviewed client files for admission agreements, physician reports, and needs and services plans. LPA reviewed staff files for first Aid/CPR certification, criminal record clearances, trainings, and health screenings. LPA observed staff#1 (S1) did not have their Tuberculosis (TB) test results on file.
Based on the observations made during today’s visit, a deficiency is being cited in accordance with Title 22, Division 6, of the California Code of Regulations.
An exit interview was conducted. Copies of reports LIC809, LIC809D, LIC9102 were provided with appeal rights to Program Director, Kragness.
