Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived unannounced to the facility in order to conduct a case management visit for the purpose of clearing Plan of Correction(s) (POC) for deficiencies that were cited on 03/30/2026. LPA met with Staff, Angelina Gomez and introduced self and stated purpose of the visit. LPA spoke over the phone with Administrator, Frank Pesqueira and informed him about the purpose of the visit. Administrator stated that he was working from home and that he was available for any questions.
LPA observed a sufficient supply of perishables/non-perishables, and a sufficient supply of towels and comforters.
No deficiencies were cited during this visit. The Plan of Corrections were all cleared during this visit.
An exit interview was conducted where this report LIC809 was discussed and a copy provided to the Administrator, Frank Pesqueira who later arrived.
