Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to Special Incident Reports (SIRs) submitted to the Community Care Licensing Office on 01/6/26 and 12/29/25. LPA was greeted and met by staff, Nancy Tate. LPA introduced self and stated purpose of the visit. Nancy phone called Administrator, Patricia Woods and informed her about the visit. Administrator, Patricia Woods later arrived and LPA introduced self and stated the purpose of the visit.
Per the two SIRs, one client lost 10 pounds within a month and another client lost 12 pounds within a month.
During today's visit, LPA met with the Patricia and Nancy to discuss the incident and surrounding events. LPA conducted an interview and record review. During record review, it was revealed that the client who lost 10 pounds was recently hospitalized and had been receiving medical care often. During interview and record review, it was revealed that the client who lost 12 pounds was placed on a weight loss plan by their doctor.
No deficiencies and one technical violation was issued during today's visit. An exit interview was conducted where a copy of this report LIC809 and LIC9102TV was provided to Administrator, Patricia Woods.
