Facility Evaluation Report
On 10/24/2025 at 1:35PM Licensing Program Analyst (LPA) Renese Howell-Small arrived announced to the facility to conduct a facility closure inspection. LPA met with Administrator, Noelle Clarke and stated the purpose of the visit.
LPA completed a walk through of the facility with the Administrator and observed the residence is empty of all of its furniture with the exception of the oven in the kitchen. LPA observed no personal belongings of clients in each of the bedrooms. LPA did not observe any clients in care during the inspection.
As of 10/24/2025, the facility will be closed, per the Licensee/Administrator's request.
An exit interview was conducted where this report LIC809 was discussed and a copy was provided to Licensee/Administrator, Noelle Clarke.
