Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced visit to the facility for the purpose of a Case Management visit health and safety check. LPA Echeverria identified herself to Staff, Justin Medina and Administrator, Lydia Escutia and discussed the purpose of the visit. No imminent health and/or safety concerns observed at the time of visit.
No deficiencies were observed during this visit. An exit interview was conducted where this report LIC809 was discussed and provided to Staff, Justin Medina.
