Facility Evaluation Report

Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced visit to this facility to discuss an incident report that was received by the Department on 06/03/25. LPA met with Staff,Tanner Golliday. The following is a summary of the incident:

On the morning 06/02/25, Staff 1 (S1) observed visible swelling and bruising on Client 1 (C1) left upper arm while assisting them with hygiene routine. S1 discovered that Staff 2 (S2) pushed C1 as a result of C1 charging S2 with feces on their hand two nights prior. During today's visit, it was discovered that S1 terminated S2 on 06/02/25 for failing to report incident and not utilizing de-escalation training.

No deficiencies were cited during today's visit. An exit interview was conducted where a copy of this report was provided to Licensee, Jeannette Phillips who later arrived.
