The inspector’s account

On 3/30/2026 at 10:00 AM, Licensing Program Analyst (LPA) Eldin Serrano conducted a tour of the facility for a complaint investigation. LPA observed that the facility gates on both side of the house is locked with a lock mechanism. Facility is not allowed to have the gates locked as per health and safety of the residents in care. Deficiency and civil penalty issued.
