Facility Evaluation Report

Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced case management visit to the facility. LPA introduced self and stated the purpose of the visit to Staff, Shontae Island. Shontae phone called Administrator Beverly Harris and informed her about the LPA's uannounced visit. Administrator, Beverly Harris arrived shortly after.

The purpose of today's visit was to deliver an Immediate Exclusion letter for an individual named David Sturdivant to Administrator, Beverly Harris. The exclusion of David Sturdivant is based on a complaint of inimical conduct.

LPA reviewed the exclusion letter with Administrator Beverly Harris, explaining that David Sturdivant is not allowed to be present in the facility. The Administrator acknowledged understanding of this Immediate Exclusion and has agreed David Sturdivant cannot be allowed to work in a CCL Licensed facility and/or have contact with clients in any facility licensed by the California Department of Social Services.

LPA verified that David Sturdivant was not present at the facility during the visit. LPA was informed by Administrator that this person has not worked at the facility since September 2, 2024 and was terminated on September 4,2024.

No deficiencies were cited during this visit. An exit interview was conducted where this report and Immediate Exclusion letter was discussed and provided to Administrator, Beverly Harris.
