Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria conducted an unannounced case management visit to follow up on an incident report sent to licensing dated on 11/05/25. LPA was greeted and explained the reason for the visit to Staff, Morena Hall and Administrator, Shanellreya Knowles.
Per the incident report, a client temporarily required the use of a catheter. Record review revealed that the facility conducted training for all staff with a licensed professional on the care and assistance for the catheter.
No deficiency was cited during this visit. An exit interview was conducted where this report LIC809 was discussed and provided to Administrator, Shanellreya Knowles.
