Licensing Program Analyst (LPA), Paola Guerrero arrived at the above location to deliver findings on complaint CONTROL NUMBER #56-AS-20231027133450. This case management visit conducted by LPA Guerrero, is being made in conjunction with complaint investigation CONTROL NUMBER #56-AS-20231027133450. The purpose of the visit is to address additional concerns during the course of an investigation of " Staff hit resident causing multiple bruising " allegation regarding C1. Based on inconsistencies in the information received through interviews false claims were made. This violates title 22 regulation "False Claims" which poses a health and safety concern.
