Facility Evaluation Report
On 04/13/2026 at 1:40PM Licensing Program Analyst (LPA) Renese Howell-Small arrived unannounced to the facility in order to conduct a case management visit for the purpose of clearing Plan of Correction(s) (POC) for deficiencies that were cited on 10/07/2025.
LPA observed three (3) clients in care and three (3) staff. LPA reviewed staff files for CPR/First Aid certification, Health screenings with Tuberculosis (Tb) result, observed a sufficient supply of perishables/non-perishables, observed emergency supplies, reviewed the Infection Control Plan and confirmed that current staff are associated to the facility. LPA also conducted a brief tour of the facility including the client bathroom.
No deficiencies were cited during this visit. The Plan of Corrections were all cleared during this visit. LPA discussed having a sufficient quantity of emergency/personal care supplies. The facility's licensing fees are current.
An exit interview was conducted where this report LIC809 was discussed and a copy provided to the Administrator, Claudia Walker.
