Allegation #3 - Administrator Espiritu Santo provided transportation logs for R1's medical appointments. Logs reveal time and dates of appointments for R1. Logs also reveal communication phone number and notes related to any changes.
Based on the information obtained there is not enough evidence to support the allegations made in this complaint. Therefore, the allegations are deemed UNSUBSTANTIATED at this time. This report was signed by LPA Prieto and Administrator Espirtu and a copy was left with the facility.
