The inspector’s account

On 10/31/2025 Licensing Program Analyst (LPA’s) Edith Conchas made an unannounced visit to the facility to conduct a required comprehensive annual inspection. LPA met with Licensee, Ann Clark, who assisted with the tour of the facility. The two (2) clients were at day program at the time of the visit

Physical Plant: The facility is a four bedroom, two bathroom home. The facility is operating in the capacity approved by Community Care Licensing (CCL). The facility is a vendor for Inland Regional Center level 4I. There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature at 77 degrees Fahrenheit. LPA inspected resident’s bedrooms; they are equipped with required furniture such as mattresses, night stands, storage space, trash bins and and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. The water was measured at 111.3 and 118.7 degrees.

LPA observed sufficient furniture and lighting throughout the facility. The facility is equipped with operating smoke detectors and carbon monoxide alarms. One (1) fully charged fire extinguisher purchased on September 25, 2025. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to residents in care in the office. Games and puzzles for activities were observed available in the living room

LPA observed there was a designated storage space for resident/staff files. Medications are kept locked in a closet in the living room area inaccessible to residents in care. Overall, the facility is clean and operating. LPA did observe broken fence in back yard. a citation was issued. Overall it is in safe conditions for residents in care.
