Facility Evaluation Report

Licensing Program Analyst (LPA) LaVette Farlow completed an unannounced Case Management visit to complete the closure of Teenee's House, facility #366405852, located at 801 S. Westeria Avenue, Rialto, CA 92376. The new business name and facility number are as followed: Teenee's Home, #365530303. LPA was granted entry by Lead staff, Thelma Agpalo. LPA Farlow explained the purpose of th e visit. Staff Thelma notified Jerry Parchejo, of LPAs arrival. LPA Farlow completed a tour of the facility, with Lead staff Thelma.

During the visit the following was observed: The facility has (3) resident bedrooms, (2) resident bathrooms, (3) staff sleeping quarters, living room, dining area, kitchen, backyard, and attached garage. During this visit, LPA Farlow observed the facility to be in good condition and there is plenty of food for the residents in care. The facility current census is three (3). Residents appear to be in normal health, and the health and safety is maintained. Upon records review an Pre-Licensing Inspection was conducted on September 11, 2025, for License #365530303. LPA collected the following documents:
