Licensing Program Analysts (LPAs) LaVette Farlow and Edith Conchas conduct an unannounced case management visit to collect a signature from a previous annual inspection conducted on 03/24/2025. LPAs met with Licensee Grace Palomares and Administrator, Raphael Lladones. LPAs reviewed the report and collected the signatures from staff.
