Facility Evaluation Report
On 8/7/2026 at 12:25 PM, Licensing Program Analyst (LPA) LaVette Farlow made an unannounced visit to the facility for the purpose of conducting a required comprehensive annual inspection. LPA was greeted and granted entry by Caregiver, Olaide Ibitoye. Olaide notified the Administrator Wesley "Monday" Idowu of LPA's arrival. LPA later met with Facility Administrator Wesley "Monday" Idowu and explained the purpose of the visit. The facility is a four (4) bedroom, two (2), bathroom home, with a kitchen/dining area, living room, and attached garage. Three bedrooms are for clients in care and 1 bedroom is designed for staff quarters. The facility is an Adult Residential Facility (ARF) capacity is (4) current census (4). Upon LPA's arrival only one (1) client was present in the facility the other three (3) clients were away at the Day Program. LPA was escorted by Olaide, Caregiver to complete a facility tour and during general inspection. During the inspection the following information was observed:
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature 78 Degrees Fahrenheit. LPA inspected client’s bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, and sufficient lighting; bathrooms were clean, and appliances were operating appropriately. LPA observed sufficient furniture and lighting throughout the facility. LPA measured and observed the water temperatures in the bathrooms to be 117.9 degrees F and the second bathroom in the master bedroom LPA was unable to get a proper reading due to the water pressure being so low, and the water shut off during the attempted reading. Two Deficiency cited. The facility is equipped with operating smoke detectors and carbon monoxide alarms. Posters such as personal rights, the CCL complaint poster, and the disaster plan were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept inaccessible to clients in care. There was a designated storage space for client/staff files. Medications are kept inside desk cabinet inaccessible to clients in care. Overall, the facility is clean, in good repair, and operating in safe conditions for clients in care.
Food Service: Non-perishable and perishable food supply is sufficient for number of clients in care.
Care & Supervision: The facility has sufficient care staff for coverage 24 hours a day, 7 days a week. All staff members working in the facility have criminal record clearance through the department.
