Facility Evaluation Report

Licensing Program Analysts (LPAs) LaVette Farlow and Edith Conchas arrived unannounced to conduct the required annual visit to the facility. LPAs met with Licensee, Ann Suzzette Sinohin and introduces themselves and stated purpose of the visit.

The facility has 4 bedrooms, 1 bathrooms, for residents in care, 1 staff bedroom with bathroom, kitchen, pantry room, dining area, living room, family room, activity room, laundry area, backyard, attached garage and 1 shed. LPAs completed a walk through of facility, review of records, P&I audit and medication audit.

Physical Plant: The facility is operating in the capacity approved by Community Care Licensing (CCL). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 75 degrees Fahrenheit. LPAs inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs and sufficient lighting. LPAs inspected client bathrooms; bathrooms were clean and appliances were found functional. Water temperatures tested at 98.4, 98.6, 101.9 and 91.9 degrees Fahrenheit. Which is below regulation. Licensee did adjust the water heater level. A technical violation cited. The facility is equipped with operational smoke detectors, carbon monoxide alarms, charged fire extinguisher and first aid kit. Posters such as; the personal rights, disaster plans, license and CCL complaint poster were posted in a common area. Cleaning supplies, toxins, sharps, and other dangerous items were kept locked and inaccessible to clients. There was a designated storage space for client/staff files. Medications was observed locked and inaccessible to clients. There is no swimming pool or bodies of water in the facility. Overall the facility is clean, in good repair, and operating in safe conditions for clients in care.

Food Service : Non-perishable and perishable food supply is sufficient. Facility has a wide variety of food available. Dishes, cups, and utensils were also stored properly.
