Facility Evaluation Report
On 07/15/2026 at 01:25 PM, Licensing Program Analyst (LPA) LaVette Farlow conducted an unannounced visit to the facility to conduct the required comprehensive annual inspection to the facility. LPA were greeted by DSP Staff, Esimaje "Harrison" Okotie and gained access to the home. LPA Farlow met with and greeted the House Manager, Nelson Wilfred and explained the purpose of the visit. LPA asked Nelson to notify the Administrator Kingsley Okundaye of LPA's arrival.
The facility has four (4) bedrooms, three (3) bathrooms, kitchen, dining room, living room, staff office, attached garage, and backyard. The facility is vendorized by Inland Regional Center (IRC). LPA Farlow completed a tour of the facility, review of records, Personal and Incidental (P&I) and medication audit.
Physical Plant: The facility is operating in the capacity approved by Community Care Licensing Division (CCLD). LPA Farlow observed three (3) clients during the visit. One client was away from the facility at the Day Program. The Facility census is four (4). There are no obstructions to indoor and outdoor passageways. The facility is maintained at a comfortable temperature of 77 degrees Fahrenheit. LPA Farlow inspected client bedrooms; they are equipped with required furniture such as: mattresses, night stands, storage space, chairs, and sufficient lighting. LPA Farlow inspected client bathrooms; bathrooms were clean, and appliances were found functional. Water temperatures tested at 119.5 and 119.3 degrees Fahrenheit. The facility is equipped with operational combined smoke detectors and carbon monoxide detectors, charged fire extinguishers, and first aid kit with first aid book.
Posters such as; the personal rights, CCLD complaint poster, Emergency Disaster plan, Infection Control Plan were available for review and updated. Client medications were kept in secure cabinets inaccessible to clients. LPA Farlow observed night lights at the hallway leading to clients' shared bathrooms. The facility had emergency kits, emergency food and water. There are no firearms and ammunition in the facility.
