Facility Evaluation Report

On 6/10/2026 at 1:39 PM Licensing Program Analyst (LPA) LaVette Farlow called the Administrator Nelson Wilfred to verifed if C1 was at the facility or the Day Program. Nelson explained that C1 was currently at the Day Program and would return between 2-3PM. At 2:32 PM, Licensing Program Analyst (LPA) LaVette Farlow called to speak to DSP Harrison and was informed that C1 has returned from the Day Program. LPA informed DSP Harrison I would be visiting the facility. LPA Farlow arrived at the facility at 2:38PM to conduct a case management visit to gather additional information, and complete an interviews regarding Complaint #56-AS-20260602141543, that the Department received on 06/02/2026. LPA Farlow completed interviews and concluded the visit for the day. An exit interview was conducted and a copy of this report was provided to Licensee, Kingsley Okaundaye.
