This is an official report of an unannounced visit/investigation of a complaint received in our office on
03/24/2026 and conducted by Evaluator Sarina Ramirez
COMPLAINT CONTROL NUMBER: 56-AS-20260324135211
Staff did not ensure a clients medication was inaccessible to other residents in care
Licensing Program Analyst (LPA) Sarina Ramirez conducted an unannounced visit to the facility to conduct a complaint investigation on the above allegation. LPA met with Administrator Artemio Dirige, and discussed the purpose of the visit.
Regarding the allegation, LPA Ramirez conducted a facility tour and interviews. Although LPA observed all three (3) clients’ medications stored centrally in a locked cabinet inaccessible to clients, interviews with Client #1 (C1), staff, and outside parties indicated that C1’s medication had been left in their room and accessible for at least ten (10) days. Therefore, based on the interviews conducted during investigation, the above allegation is Substantiated.
A finding that the complaint is Substantiated means that the allegation is valid because the preponderance of the evidence standard has been met.
An exit interview was conducted where this report LIC 9099, LIC 9099D and Appeal Rights were discussed, and a copy was provided to Administrator at the conclusion of the visit.
