Facility Evaluation Report

On 07/21/2025 at 9:00 AM, Licensing Program Analyst (LPA) Eldin Serrano arrived at the facility unannounced to conduct a Case Management Visit for health and safety. This case management visit is in response to an email received at the Community Care Licensing Office on 07/15/2025 from Central Application Bureau (CAB). LPA was greeted by Administrator Loreta Tuparan at the front door. LPA introduced self and stated purpose of the visit.

During today's visit, LPA discussed the purpose of the visit to administrator Loreta Tuparan. LPA did a health and safety check and interviewed the administrator. The administrator stated that she already responded to the email sent by the regional office and explained the intent to sell the business and property of her other facility (855 Frontier Ave. Redlands, CA 92374) and not this facility. Administrator stated that it was just an intent to sell and no actual purchase agreement signed by the licensee/administrator Loreta Tuparan.

No deficiencies were observed during this visit. An exit interview was conducted where this report was, reviewed, discussed, and then provided to Loreta Tuparan.
