The inspector’s account

On April 10, 2026 (04/10/2026), Licensing Program Analysts (LPAs) Andrew Martinez and Edith Conchas conducted an unannounced visit to complete an annual facility inspection. LPAs met with Administrator Rachelle Wheateon and explained the purpose of the inspection. The facility is an Adult Day Program (ADP) with a licensed capacity of forty-five (45) clients, current census of two (2), neither in attendance today. Today's visit conducted of a facility inspection tour and record reviews (e.g. facility, staff, resident(s)) .

Inspection tour included but was not limited to the following:

Indoor and outdoor passageways are clear and free of obstructions. LPAs observed no swimming pools or similar bodies of water on the grounds. The outdoor activity spaces are enclosed, protected from traffic, with sufficient seating and shaded areas for clients in care. Indoor and outdoor activities include arts and crafts, outdoor walks, fitness and meditation exercises. The ADP provides snacks and refreshments. One (1) bedroom (Room 408) is designated as the program's "Day Break" room, located in the Seaside Villa area, equipped with a bed and bathroom operating in good and sanitary conditions. LPAs observed the hot water temperature measured within regulation requirements. Telephone service, fire alarms, and carbon monoxide alarms were tested and are in working condition. Facility fire and disaster drills were conducted in February and March 2026. The facility has sufficient personal protective equipment. Disinfectants and other toxic supplies are kept locked and inaccessible to clients in care.

Staff files reviewed were observed to be complete. Client files reviewed were observed to be complete.

No deficiencies were cited during today's visit. An exit interview was conducted, and copy of this report was discussed with/provided to Administrator, Rachelle Wheaton at the conclusion of the visit.
