The inspector’s account

Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Administrator Annalisa Sialana, and discussed the purpose of the visit.

The facility is an Adult Day Program with a license capacity of (30) and a current census of (23) clients. LPA conducted an overall inspection, which included, but was not limited to the following:

The program facility is in good repair and operating at a comfortable temperature. Facility passageways are clear and free of obstructions. Facility has sufficient space for client activities. Client activities include arts and crafts, board games, dancing, karaoke, watching movies. LPA inspected three (3) client bathrooms. Bathrooms are operating in a clean and sanitary condition.The hot water temperature in bathrooms tested between 110.3 to 113.1 degrees F. LPA inspected facility kitchen. Kitchen is clean and free of odors. Snacks are stored in a healthful manner. Chemicals and Sharps are kept locked inaccessible to clients. The facility has operating laundry equipment. Posted in a common area: Emergency telephone numbers, evacuation plans, personal rights, activities schedule, cleaning duties, and CCLD compliant poster. Last facility disaster drill was conducted on 07/08/26. Facility has a complete first aid kit. Five (5) staff files were observed to be complete. Five (5) client files were observed to be complete. Two (2) Client’s medications are labeled and centrally stored in a locked cabinet and administered as prescribed.

Based on observations and record review, one (1) deficiency for not having an infection control plan and technical assistance for carbon monoxide alarms will be cited per Title 22 of the California Code of Regulations. An exit interview was conducted, the licensing reports were discussed with Administrator. A copy of the reports (LIC 809, LIC 809D, LIC 9102) and appeal rights were provided to Administrator Annalisa Sialana at the conclusion of the visit.
