Facility Evaluation Report
On 11/18/25 Licensing Program Analyst (LPA) Sarina Ramirez conducted an unannounced case management visit. The purpose for this visit is in response to the facility's request for a capacity increase from a census 20 to 40. LPA Ramirez met with Administrator Delcie Mucha who assisted LPA with the facility tour.
Per Mucha the Redlands Fire Department conducted the fire clearance tour on 11/06/25, however Mucha is still waiting for the approved STD 850 form.
LPA observed the newly built addition unit, LPA observed no residents residing in the new unit . As the facility accepts new residents for the new unit staff will include two (2) caregivers and one (1) med technician.
LPA observed a dining area with adequate seating for 20 people and furniture is in good repair. The kitchenette is connected to the dining area. LPA observed a room where the medication will be centrally stored inaccessible to residents. LPA also observed oe (1) storage closet.
LPA inspected ten (10) shared bedrooms with restrooms and two additional (2) shared bathrooms. LPA observed a few rooms to have new mattresses however; some rooms are not fully furnished due to still no residents. All bathrooms had handrails. LPA observed that the model rooms had required bedding and furniture such as new mattress/linen, sufficient storage space, chairs and lighting. LPA measured the water temperature in two (2) bathrooms measuring between 106.2 to 120 degrees Fahrenheit
No deficiencies we cited during this visit. An exit interview was conducted where this report was discussed and provided to Administrator Delcie Mucha
