Facility Evaluation Report
On 3/18/2026, Licensing Program Analyst (LPA) Sarina Ramirez made an unannounced visit to the facility to conduct a health and safety check. LPA met with Administrator, Regina Chavez, and explained the purpose of the visit.
During today's health and safety check. LPA conducted an overall status of the facility. At the time of the visit there were three (3) staff present. The facility was observed to have operable utilities (gas, water, electricity). The facility was observed to have a 2-day supply of perishable and a 7-day supply of nonperishable food items. The facility was a comfortable temperature.
Records review the facility does not have an active liability insurance, deficiency issued.
Deficiencies were cited during LPA's visit. An exit interview was conducted, and a copy of this report was provided to Administrator, Regina Chavez
