Facility Evaluation Report

On 3/11/2026 at 9:10 am, Licensing Program Analyst (LPA) E.Conchas made an unannounced visit to the facility to conduct a health and safety check. LPA met with administrator, Regina Chavez, and explained the purpose of the visit.

During today's health and safety check LPA conducted an overall status of the facility. At the time of the visit there were two (2) staff and (4) residents present. The facility census has not changed. LPA observed the residents eating breakfast. The facility was observed to have operable utilities (gas, water, electricity). Water temperature measured between 115-120 degrees Fahrenheit. LPA observed liability insurance was current. The medications were observed to be locked and inaccessible to the clients in care. The facility was observed to have a 2-day supply of perishable and a 7-day supply of nonperishable food items.

No deficiencies were cited. An exit interview was conducted, and a copy of this report was provided to administrator, Regina Chavez
