Complaint Investigation Report
Regarding the allegation staff did not provide adequate food service: Interviews conducted all clients are required to bring their lunches and snacks, however if a client forgets to bring their lunch or snacks the facility will provide. LPAs observed a supply of food and snacks stored at the facility for the clients who do not have a lunch.
Regarding the allegation staff did not meet reporting requirements: Interviews conducted with facility staff and tour of the facility there are no witnesses or evidence to corroborate the allegation.
Regarding the allegation staff did not treat client with respect: Interviews conducted with facility staff all denied not treating clients with respect. LPAs were unable to interview clients due to their inability to communicate. There were no witnesses or evidence to corroborate the allegation.
Regarding the allegation staff did not meet buildings and grounds requirements: LPAs observed the facility to be clean and in good repair, there were no foul odors, bathrooms were observed to be clean and sanitary. There was no clutter or obstruction in hallways or walkways. No evidence to corroborate the allegation.
Based on LPAs observations, record review, and interviews, the above allegations are Unsubstantiated; meaning that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur.
An exit interview was conducted where this report was discussed and a copy with appeal rights was provided to Administrator Esquero at the conclusion of the visit.
