Facility Evaluation Report
Licensing Program Analyst (LPA) LaVette Farlow conducted an announced visit to conduct the Pre-licensing inspection. LPA met with Licensee, Emer Gatchalian, and Venancio Gatchalian. The fire clearance was approved on 12/19/2024 for five (5) nonambulatory residents and one (1) bedridden resident.
The facility has four (4) resident bedrooms, two (2) resident bathrooms, living room, kitchen, dining area, attached garage and back yard. LPA toured the interior and exterior areas of the facility. The following were inspected:
Resident Bedrooms : All bedrooms have the required bedding and furniture, such as, clean mattresses/linen, night stands, dressers, chairs, and lighting.
Resident Bathrooms : The bathroom appliances were operating in safe and sanitary condition. The bathroom temperature was measured 126.8, which is out of regulation. LPA advised licensee to adjust the temperature and conduct and monitor the temperature prior to accepting residents in care.
Kitchen and Dining Areas : Utensils and dishware are in good repair and ready for resident use. Kitchen appliances and counter top were in good repair. The water temperature was measured at 123.9 degrees Fahrenheit. LPA advised licensee to adjust the temperature, and monitor the temperature prior to accepting residents in care.
