Facility Evaluation Report

Community Care Licensing (CCL) staff met with licensee representative, Oyewole Joseph Jose. The meeting was held with CCL staff, Regional Manager (RM) Leslie Mendiveles, Licensing Program Manager (LPM) Karen Clemons and Licensing Program Analyst (LPA) Javier Prieto. Also in attendance Department of Labor (DOL) staff Investigator Calvin Carlyle, Community Outreach Specialist Filiberto Quintero and Assistant District Director Rafael Valles. Inland Regional (IRC) Staff Genii Greco and Vince Toms (via TEAMs) were also in attendance.

The purpose of the meeting was to provide licensee with results of DOL complaint investigative findings. Licensee agreed to meet with DOL for additional follow-up needed.

In addition, RM discussed with Licensee responsibility to ensure compliance with Title 22, Title 17, Health and Safety, Fire Department requirements, and other State and Federal Laws, including Labor. Licensee acknowledged understanding.

Based upon information received this date, the following is requested and will need to be submitted to CCL by 3/31/2026:

2) Proof of current administrator including name of administrator, copy of current administrator certificate, personnel record.
