Complaint Investigation Report
Facility ratio was met during time if incident and today's investigation. The walk from transport to the facility and back did not pose any danger to the clients and the clients were being observed by the Administrator while the incident occurred to assure supervision was met. Program Director Hill has assured that re-training on the subject of "Supervision of Individuals" will be conducted during today's visit to all staff. The incident that occurred did not pose a danger to client's in care. Video footage, and Administrator interview, reveal that supervision of clients was met. A copy of the video footage was obtained during today's inspection.
Based on the information obtained there is not enough evidence that staff left clients unattended. Therefore, the allegation is deemed UNSUBSTANTIATED at this time. this report was signed by LPA Prieto and Administrator Lopez and a copy was left with the facility.
