Facility Evaluation Report

The facility has Changing Rooms (6), a Quiet Room, several serenity rooms. The facility has a six bathrooms and a total of six bathroom changing rooms in the male and female bathrooms. The facility has an area for the clients belongs and individual lockers for the staff.

Five clients files were reviewed today. The hot water temperature measured 106.0 degrees in one client's bathroom. The smoke/carbon monoxide alarms are wired into the building and are in working order. The last fire drill was conducted on 12/10/2025 and the last disaster drill was conducted on 10/16/2025. No money is handled at the facility. The facility uses a bus for transportation and the maintenance is performed by an independent automotive repair shop. The First Aid Kit and Manual are complete and up to date. The facility's staff ratio is 1:3. Eight (8) client files staff files were reviewed today Staff Criminal Record clearances were received prior to their presence in the facility.

Based on this inspection, per Title 22, no deficiencies were observed at this time in the areas evaluated.

An exit interview was conducted with Mr. Panowicz and a copy of this report was left at the facility.
