The inspector’s account

Licensing Program Analyst, Amber Coleman arrived at the C & T Home Care Facility to conduct a Pre-Licensing visit in response to the Administrator's change of ownership. The facility was initially licensed to Criszelie Garcia Facility No. 361800191 and will be licensed to RHE Management, Inc. Criszelie Garcia will continue to be the Administrator. LPA was greeted upon and granted entry into the facility by Criszelie Garcia. LPA introduced self and stated the purpose of the visit. Administrator stated that the residents are currently at their Adult Day Program. Administrator indicated that the phone number for the facility has changed to: 909-244-9610 and this is the landline to the facility itself.

During the visit, LPA and Administrator walked through the facility, reviewed resident's charts and interviewed Administrator. Facility file was reviewed prior to the visit.

This is an existing facility with a capacity of 6, Ambulatory clients only, ages 18-59. There are currently four (4) residents in care at this time. The facility is currently operating and is pending change of ownership.

The facility is a one story, 5 bedroom, 3 bathroom residence. (3) bedrooms are designated for client use, (2) bedrooms are designated for staff use. Two residents have their own room while 2 residents share a room. The facility consists of a living room, family room, kitchen, laundry room, dining area, storage, and backyard. The facility has a shaded patio area that is free of hazards and accessible to residents. Infection control, house rules and Licensing Information was observed posted throughout facility.

There are 5 adequate smoke and carbon monoxide detectors mounted throughout the facility. Medications, sharp objects and medications are stored securely in a file cabinet in the kitchen. Poisons, toxic substances, all dangerous chemicals were stored securely in the kitchen under the sink. Hot water temperature was measured at 110.6 degrees. Linen supply, furnishing, cleanliness and state of repair, toilets/sinks/showers/tubs were observed with no abnormalities. Facility chairs, night stands, and reading lamps in each room all included adequate drawer and closet space for each resident's belongings. Emergency exit(s) were observed labeled and free of obstruction. Fire extinguishers last inspected August of 2022.

During review of resident files, LPA reviewed 4 resident files. Of those 4 resident files, 2 residents were deemed "Non-Ambulatory"

The facility was evaluated in accordance with the CCR, Title 22, Division 6, Chapters 1 and 6 to ensure the health and safety of clients in care. Based on the observations and evaluation of the facility this date, the facility must make the corrections prior to being licensed.

An exit interview was conducted and a copy of this report was left with the Licensee - Criszelie Garcia
