Facility Evaluation Report
Licensing Program Analyst (LPA) Paola Guerrero arrived at the facility to deliver investigative findings. LPA met with Assistant Administrator Alice Reyes and explained the purpose of the visit regarding the allegation stated above.
On 10/14/2025 the department received a Special Incident Report (SIR), reporting that on 10/12/2025 Staff#1 allegedly sexually abused Client#1
The investigation shows the department conducted interviews with Staff #1 and Staff#2 regarding the alleged allegation. Staff #1 and Staff #2 denied the allegation. Staff #1 informed the department that Client #1 underwent a minor surgical procedure that required aftercare and cleaning of the private area. S#1 further explained that the touching of the area was due to aftercare instructions and not sexual abuse towards Client #1.
The department conducted an interview with Client#1 regarding the alleged allegation. Throughout the interview the department found that there was inconsistency with the details Client #1 provided compared to the details Client#1 had initially reported. In addition, it appears that Client #1 was motivated to make false allegations towards Staff #1 Due to the lack of evidence provided the department found that the allegation of sexual abuse is Unsubstantiated.
Unsubstantiated; meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur.
An exit interview was conducted where this report was discussed, and appeal rights were provided toAssistant Administrator Alice Reyes.
