The inspector’s account

Licensing Program Analyst (LPA) Raquel Hernandez conducted an unannounced Case Management incident visit. LPA met with Program Director Art Estrada and explained the reason for today’s visit. The visit is pertaining to incident that occurred on 06/07/2025 pertaining to Client #1 (C1).

During visit, LPA conducted (1) staff interview and (1) client interview. Based on interviews, LPA did not observe any health or safety concern for clients in care.

During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations regarding this incident. An exit interview was conducted and this report was provided to Program Director Art Estrada.
