Second Allegation :-Staff do not allow clients to use restroom.

Based on the investigation by LPA Singh, the allegation that Staff do not allow clients to use restroom or use restroom was not supported by any evidence. All five (5)clients out of five clients and five (5)staff out of five staff members interviewed consistently stated that clients are always allowed to use the restroom whenever they need to while at the day program.

Third Allegation: - Staff force client(s) to stay in room.

Regarding the allegation the Staff force client(s) to stay in room; LPA Singh interviewed Five (5) Clients and five (5)Staff. Five(5) out of Five (5) Staff and Five out of five clients stated Staff do not force client(s) to stay in room at the day program and never ever force clients to stay in the room. There was no witnesses testifying that Staff force client(s) to stay in room.

Fourth Allegation :- Staff interact inappropriately with clients.

Regarding the allegation the Staff interact inappropriately with clients; LPA Singh interviewed Five(5) Clients and five (5)Staff. Five(5) out of Five (5) Staff and Five out of five clients stated Staff do not interact inappropriately with clients at the day program and stated that all Staff behave professionally towards the Clients. There was no witnesses testifying that Staff interact inappropriately with clients.

Based on the information obtained there is not enough evidence that Staff withhold food from clients, Staff do not allow clients to use restroom, Staff force client(s) to stay in room or Staff interact inappropriately with clients at the day program. Therefore, the allegations are deemed UNSUBSTANTIATED at this time. A finding that the complaints are UNSUBSTANTIATED means although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur. During today’s visit, no deficiencies were cited per Title 22, Division 6, of the California Code of Regulations.

An exit interview was conducted where this report LIC9099,LIC9099C was discussed and was provided to Facility Program Director Tiffany Johnston by the end of the visit.
