Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to a Special Incident Report (SIR) submitted to the Community Care Licensing Office on 02/05/26 by the facility. LPA was greeted and met by Program Supervisor, Audriana Mathis at the front door. LPA introduced self and stated purpose of the visit. LPA met with Administrator, Ana Vilchis and stated the purpose of the visit.
On 02/04/26, staff 1 (S1) reported to management that staff 2 (S2) was observed sleeping for approximately 5 hours during their shift.
During today's visit, LPA discussed the incident and surrounding events with Administrator and Program Manager. Administrator stated that care and supervision was provided by S1 during the incident. Records review revealed that S2 was placed on unpaid leave for investigation and a meeting was held with staff covering training topics: Preventing Abuse-Mandated Reporter-Zero Tolerance, Mandated Reporting (video), Mandated Reporting Requirement, Corrective Action Policy, Obligations to report Abuse/Neglect, Health & Safety and Supervision of Individuals Acknowledgement.
No deficiencies were issued during this visit. An exit interview was conducted where this report LIC809 was, reviewed, discussed and then provided to Administrator, Ana Vilchis.
