Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to a Special Incident Report (SIR) submitted to the Community Care Licensing Office on 12/16/25 by Inland Regional Center (IRC). LPA was greeted and met by Staff Javier Garza and Melissa Serrato at the front door. LPA introduced self and stated purpose of the visit. Program Supervisor, Audriana Mathis later arrived and LPA introduced self and stated the purpose of the visit.
On 12/15/25 morning, client 1 (C1) wheelchair tipped backwards inside the moving van during transportation to program. Per report received, staff 1 (S1) reported that the seat belt straps loosened.
During today's visit, LPA met with Program Supervisor, Aduriana Mathis to discuss the incident and surrounding events. LPA interviewed staff and reviewed records. During records review, it was discovered that S1 was placed on unpaid leave for investigation. It was also discovered that the facility did not report to Community Care Licensing (CCL) and only reported to IRC.
Deficiencies were issued during this visit. An exit interview was conducted where this report LIC809, LIC809D and appeal rights were, reviewed, discussed and then provided to Audriana Mathis.
