Facility Evaluation Report
Licensing Program Analyst (LPA) Michelle Echeverria arrived at the facility unannounced to conduct a Case Management Visit. This case management visit is in response to a Special Incident Report (SIR) submitted to the Community Care Licensing Office on 05/12/26 by the facility. LPA was greeted and met by Staff Javier Garza at the front door. LPA introduced self and stated purpose of the visit. LPA also greeted and informed Program Supervisor, Audriana Mathis about the purpose of the visit.
On 05/10/26, the facility's program director, Ana Vilchis observed that a referral was made for client 1 (C1) during their hospitalization about an allegation against staff 1 (S1).
During today's visit, LPA spoke to Program Director, Ana Vilchis over the phone and with Program Supervisor, Audriana Mathis to discuss the incident and surrounding events. LPA conducted interviews and reviewed records. It was discovered that S1 was placed on unpaid leave for investigation.
No deficiencies were issued during this visit. An exit interview was conducted where this report LIC809 was, reviewed, discussed and provided to Program Supervisor, Audriana Mathis.
