Facility Evaluation Report
Licensing Program Analysts (LPAs) Eldin Serrano made an unannounced visit to the facility to conduct a required annual inspection. LPA met with Program Director Annabell Hernandez, and discussed the purpose of the visit.
The facility is an Adult Day Program with a current census of 46 clients. During today's visit, there were 17 direct care staff present. LPA conducted an overall inspection, which included, but was not limited to the following:
The facility's passageways were clear and free of obstructions. The facility has five (5) areas designated for client activities. The facility has sufficient supply of arts supplies, board games, sensory activity items for client use. Client restrooms were odor free and operating in sanitary conditions, hot water temperature measured at 105.2 degrees Fahrenheit. Cleaning solutions stored in a supply closet kept locked and stored away from clients in care. No sharps on site and no meal provided at the facility. They just have emergency food just in case the client forgot their lunch food bag from home. The facility has operating fire station alarm, smoke detector and carbon monoxide alarms. The facility has posted in a common area: personal rights, facility license, facility evacuation sketch, Community Care Licensing complaint poster, and an emergency disaster plan with emergency telephone numbers. Four (4) staff files were reviewed and included criminal record clearances. LPA observed that staff#2 (S2) has a tuberculosis (TB) test result that doesn't show result of negative. Technical violation issued. Four (4) client files were reviewed. LPA observed that client#1 (C1) needs an updated tuberculosis (TB) test results. Technical violation issued.
LPA observed that all of the facility vehicles has the emergency disaster food and water as well first aid kit.
