Based on the information obtained there is not enough evidence that staff do not ensure facility is in good repair and staff do not ensure a clean and sanitary environment . Therefore, the allegation is deemed UNSUBSTANTIATED at this time. This report signed by LPA Prieto and Administrator Santos and a copy was left with the facility.
