Facility Evaluation Report
On March 2, 2026, at 12:15 PM, Licensing Program Analyst (LPA) Eldin Serrano arrived unannounced at the facility to conduct a Case Management Visit in response to a self-reported incident received by the Community Care Licensing Office on February 26, 2026. LPA was greeted by Administrator Anna Marie Santos, introduced himself, and stated the purpose of the visit.
During the visit, LPA interviewed residents regarding the allegation that a staff member borrowed money from a resident. As the staff member is no longer employed at the facility, the allegation cannot be verified. LPA also investigated a report that a resident sold narcotics to another resident; interviews with relevant parties revealed no witnesses, and this allegation could not be substantiated. A third allegation, involving staff disclosing personal information about other residents, could not be verified due to the unavailability of individuals involved.
No deficiencies were observed during this visit. An exit interview was conducted during which the report was reviewed, discussed, and provided to Administrator Anna Marie Santos.
