Facility Evaluation Report

Community Care Licensing (CCL) and Inland Regional Center (IRC) staff met with licensee representative, Lilian Tolentino, and staff Cristina Pastorfide. The meeting was held with CCL staff, Regional Manager (RM) Leslie Mendiveles, Licensing Program Manager (LPM) Karen Clemons and Licensing Program Analysts (LPAs) Raquel Hernandez and Eldin Serrano. IRC staff in attendance were Genii Greco and Dahlia Balderas.

The purpose of the meeting was to discuss Department of Labor (DOL) investigation and obtain proof of resolution. CCL and IRC staff, along with licensee, discussed the nature of the investigation. According to licensee, there was a judgement and order finalized around 2/11/2025. Licensee provided CCL copy of the order along with proof of ongoing payments being made. Licensee reported that judgement will be settled in April 2028.

In addition, the following documents were received by licensee:

Following discussion, Licensee will do the following:

1. By 11/25/2025, submit updated Lic 200 and administrator documents for current administrator as required.

2. Technical Support Program (TSP) services was offered to licensee who accepted. Referral will be generated from Regional Office.

In addition, RM discussed with Licensee responsibility to ensure compliance with Title 22, specifically reporting requirements, Health and Safety, and other State and Federal Laws, including Labor. Licensee acknowledged understanding. Upon completion of meeting, Licensee reviewed and signed report and copy was provided.
